In wordy way:

Start with a slip knot and make two chains.

Then make 6 double crochets (DC) in the first chain you made. Put a stitch marker in the first DC made, move this marker in each future row marking the start of the round.

In the next row make 2 DC in each stitch in the round. You should have 12 stitches at the end.

For the next row repeat the sequence 1 DC in first stich and 2 DC in second stitch, making an even increase across the row. At the end of the round you will have 18 stitches.

Then decide which side of the circle is the top and work a DC into the top loop of each stitch (rather than both of the loops of the stitch).

For the next two rows double crochet all the way around.

The next sixrows decrease the shape to form the stalk of the mushroom. To avoid weaving in the tail make sure its on the top side fo the circle!

First make 7 DC stitches then decrease (preferably an invisible one) twice and complete the round with 7 DC. You should have 16 stitches.

Second make 6 DC stitches then decrease twice and complete with 6 DC. You should have 14 stitches.

Third make 5 DC stitches then decrease twice and complete with 5 DC. You should have 12 stitches.

Fourth make 2 DC stitches a decrease, 4 DC, a decrese and finish with 2DC. You should have 10 stitches.

Fifth do the following twice: 3 DC and a decrease. You should have 8 stitches.

Sixth make 1 DC stich a decrease, 2 DC, a decrease and finish with a DC. You should have 6 stitches.

That completes the stalk, now we want to make the underside of the cap. To do that we want to make another circle from where we are now. To do this we will:

Increase by doing 2 DC in each stitch to get 12 total stitches

In the next round repeat the following six times, make 1 DC in the first stitch and 2 DC in the second stitch. You should have 18 stitches.

Lastly repeat the following six times, make 1 DC in the first stitch, 2 DC in the second and 1 DC in the third. You should have 24 stitches.

At this point we can start making the cap, so switch to your chosen colour and make DC in each top loop all the way round.

Then make a DC in each stitch normally

In the next round repeat the following six times, 1 DC in the first stitch, make a decrease joining the second and third stitch together, 1DC in the fourth stitch, you should have 18 stitches.

Next DC in each stitch in the round

Another decrease round repeat the following six times, 1 DC in the first stitch and decrease by joining the second and third stitch together. You should have 12 stitches

And another round of DC all around

And the last decrease round make a decrease in each pair of stitches all the way round, you should have six stitches.

Then pull a yarn through each of the last six stitches and pull tight to close. Once done cut the yarn (leaving a decent tail) and pull through the loop on the hook to close. Weave that tail in or hook it through the middle of the mushroom to hide it.
